What you will be responsible for: Providing robust assessment of patients (by telephone or face-to-face) who have been referred to the CMHT due to presenting with mental health difficulties.Identifying appropriate care pathways and ensuring the least restrictive option to meet the needs of the patient and ensuring their wellbeing.Signposting to appropriate care providers (including primary and tertiary care). Attending daily Red Board meetings.Raising concerns to senior staff for support and management.
