Main duties and responsibilities Actively participate in the development of the organisations planned and unplanned community Hospice at Home model of care.To work as part of the multidisciplinary team to provide a high standard of individualised patient/family care and support.To be responsible for those patients allocated on a daily basis.To ensure that excellent standards of support and clinical practice and care are achieved.Deliver specialist palliative nursing care to patients requiring planned and unplanned care.
