Randstad are recruiting for a Qualified Social Worker to join Bolton Council's Safeguarding team. This is a full time role with a fantastic pay rate of £30 per hour.

Responsibilities:

• Undertake Care Act, 2014 assessments.
• Assist people to manage risk, to identify person-centered outcomes, and to undertake support planning and reviews.
• Deliver the service in accordance with statutory responsibilities set out in NHS and social care legislation and the eligibility criteria for services as agreed by the Council.
• Undertake mental capacity assessments and chair best interest meetings.
• Support carers in their roles.
• Ensure that reviews are carried out in a timely and person centered way evidencing need for continuing support.
• Provide information and advice to help people stay healthy, plan for their future needs, use services effectively, take responsibility for their own health and care, and maximise the impact of prevention and recovery services
• Encourage and support people to look after themselves, their families and their communities promoting choice and control for people.
• Proactively support and actively contribute to the delivery of integrated health and social care services.
• Build effective relationships with health partners and the voluntary and community sector.
• Promote and safeguard the welfare of adults at risk.
• Manage a complex caseload.
• Input and maintain records.
• Duty.

Requirements:

• Social Work Degree or equivalent
• Social Work England registered
• Enhanced DBS
