Connect2Dudley currently have an exciting opportunity for an experienced Assistant Care Coordinator to join the Access and Prevention team with Adult social services at Dudley MBC.

Hours - 37 hours per week

Location - Brett young carers hub, Halesowen, B63 3TB

Duration - Initially 4 months but with the potential to be extended

Pay - £13.69 to £14.91 per hour

Please note that this is not a hands on role delivering care. It is mainly a role completing Social care assessments so experience in a similar role is vital.

As an Assistant Care Coordinator within the Carers Network Team, your role will primarily be to carry out carers assessments, providing interventions that will maximise the independence and wellbeing of individuals accessing the service.

The role will involve travelling across the borough, to ensure the consistent delivery of an effective service. You will also be required to work from the Carers hub on a weekly base, as part of a Duty system for referrals into the service.

The enquires and assessments will be varied in complexity and needs and will involve working with a wide range of client groups. You will work closely as part of a multi disciplinary team with occupational therapy, social work teams, Care Assessment teams, the Multi Agency Safeguarding Hub (MASH), Dudley Disability Service, Emergency Services and NHS partners

You need to work in a person centred way with carers and their families to prevent and delay future needs whilst maximising the carers and the person they care for's own strengths and networks, ensuring public resources are used effectively and efficiently to reduce the risk of carer breakdown/crisis.
