Adults Social Worker

Team: Integrated Discharge Team

Location: Bolton

Start Date: ASAP

Contract: On going

Hours: 37 and HYBRID WORKING

Rate: £30 per hour

An Adults Social Worker is required for the Integrated Discharge Team in Bolton.

Responsibilities:

• MDT working
• Be able to complete Care Act 2014 Assessments
• Have confidence in completing Mental Capacity and Best Interest assessments
• Be able to manage and respond in a fast-paced environment
• Prioritise case load and be adaptable to re-prioritise as service allows
• Have awareness of Discharge pathways and models
• Knowledge of Liquid Logic system would be desirable
• Complete duty function as part of a rota

• DIPSW or equivalent qualification in Social Work
• Social Work England registration
• Post qualification experience
• Enhanced and in date DBS certificate.
• Excellent communication skills and be able to make good decisions based on information available.
• One year minimum post qualified experience in Adult’s Social Work
• Can work alone and within/contribute to a team/multi-agency set ups.
• Possess knowledge of service needs.
• Competent IT skills and be able to navigate electronic file/recording systems.
• Can write clear, concise, and robust assessment/reports/files.
• Have a flexible approach to work and working hours when the need arises
