Regional Coordinator for Health Checks & Follow Up

Band: Band 6

Responsible to: Project Lead

Accountable to: Project Director

Responsible for: Directly manages team, responsible for day to day work assigned to Project Team whilst on allocated project

Location: Camden, the post holder may be required to work at any establishment at any time throughout the duration of their contract.

Job summary:

• To coordinate Health Checks Follow up and Outreach services in local and community settings across Camden area.
• To liaise with the Project Lead regarding operational plans for delivery of the service.
• To providing weekly checkpoint reports detailing achievements and any issues or concerns that could impact on the service.

Essental Requirements:

• Degree level and/or equivalent, preferably in a health, science or social science field.

• 2 years experience (minimum) of project coordination, evaluation and report writing.

Desirable:

• RGN/RCN qualification
