Our client, a Psychiatric Hospital based in Sheffield, requires a Referrals Coordinator to
process referrals, arrange assessments, monitor admissions and discharges and provide
management information to support the Contract Compliance and Business
Development functions.

Your Main Responsibilities will include:

  • To receive calls from NHS partners regarding referrals to services
    at the Hospital
  • To ensure referrals follow the protocols agreed with the NHS.
  • To establish that funding has been identified for potential
    patients.
  • To liaise with clinical staff at the hospital to book assessments.
  • To chase up assessment reports ensuring that timescales are
    adhered to.
  • To collate assessment documentation and publish in the corporate
    format.
  • To monitor bed availability and flag up issues of concern to the
    Group Contract Compliance Lead and/or Head of Business Development.
  • To provide weekly updates at the Referrals, Admissions and
    Discharge Meeting.
  • To report on enquiries and referral activity on a monthly basis to
    inform monthly business development report.
  • To gather information from the handover meeting with regard to
    admissions, assessments and discharges.
  • Monitor and document progress of admissions and discharges.
  • Prepare documentation required for admission.
  • Liaise with Mental Health Act, CPA and multidisciplinary teams
    ensuring they have the information they need prior to admitting a patient.
  • To produce additional reports as requested by the Head of Business
    Development and the Group Contract Compliance Lead.

As you will be based on the Hospital wards, you must be able to deal with issues of a
sensitive and sometimes distressing nature with a mature attitude. Ideally you
will have experience within a mental health or adolescent care environment.
