Qualified Care Manager - Working With Adults

Our client is currently seeking a Qualified Care Manager with statutory experience working with Adults. It is essential that you hold a relevant Social Work qualification and are registered with the GSCC. You must have a minimum of 2 years post qualifying experience working with clients with Learning Disabilities.
Responsibilities and requirements to include:
- Assessing client’s needs and reviewing care packages in line with FACS criteria.
- Putting care packages in place within a value for money framework/timely
- Carrying out outcome base reviews
- Able to identify and applying for Continuing Care funding and joint working with health care professionals and submitting cases to Community Care Panel
- Understanding and experience in working within a multi disciplinary frame work
- Safeguarding investigations
- Carry out needs lead assessments necessary to facilitate and to safeguarding clients with the local community, or only where appropriate into residential placements, timely discharge from hospital and set up urgent care package etc.
- Abilities to assist in ensuring the availability and accuracy of resource information for people with learning disability in line with budgets requirements and presenting cases to funding Risk Panel takes place in a timely manner.
- Specialist knowledge and expertise that promotes independence, for eg. ILF, individual budgets and support planning.
- Ability to write clear assessment reports, other reports. Have good IT skills etc.
