Job Description
We are currently recruiting for an Intermediate Care Social Worker based in Cheshire to work as part of a busy team.
The candidate must have previous experience of working within adult services dealing with multi disciplinary settings. Within this role, you will have dealings with health professionals including OT's, physio, nurses and care workers.
Intermediate care teams provide a short term rehabilitation service following illness or accident to adults, generally aged over 60, but with younger adults considered on an individual basis.
The service is provided in the person’s own home wherever possible. Where this is not possible the service has intermediate care beds in local residential and nursing homes.
The service promotes faster recovery from illness, avoids inappropriate hospital admission, supports earlier discharge from hospital, avoids the need for long-term care and maintains independence.
Once in an assignment, we offer regular contact, visits, regular progress reports with your line manager, training opportunities and also the assurance that we can guarantee you 336 hours per Year.
