Qualified Social Worker - Hospital Discharge - Merseyside
A Qualified Social Worker is required to join a Hospital Discharge team in the Merseyside area.
Working within the Hospital Discharge team, this Qualified Social Worker will have the following responsibilities:
- Visiting and interviewing clients to make assessments.
- Assessing priorities and writing reports to develop care plans.
- Accepting responsibility for actions in relation to the liberty or safety of clients in emergency situations.
- Including carers and clients' families in the assessment process in line with the Equal Opportunities Act 2004.
- Liaising with colleagues and other agencies as appropriate including domiciliary, day and residential services.
The successful Qualified Social Worker will be GSCC registered, have a current CRB and 2 years post qualification experience. Knowledge of relevant legislation including Delayed Discharges Legislation 2003 would also be advantageous.
