Job Description

Job Title: Health Check Advisors

Band: Indicative Band 2

Hours: Up to 40 hours per week (we have 3 positions both full time and part time)

Location: Medway

Responsible to: Project Coordinator

Accountable to: Project Lead

Job summary:

  1. To carry out NHS Health checks in accordance with the national programme led by the Department of Health. This will involve:
  2. Accurate and timely recording of medical notes/consultations: The post will also include administrative duties:
  3. Attend and participate in project meetings as required
  4. Ordering and display of Health Promotion materials in conjunction with client participation group.
  5. To participate in any training programme implemented by the project as part of this employment, such training to include:
  6. Clinical governance responsibilities for own and colleagues quality and performance, e.g. :
  7. Networking and sharing of information/best practice:
    1. Effectively manage own time, workload and resources to meet the needs of the clients and service.
    2. Work across Medway in venues of all types and with the widest range of stakeholders to generate utilisation of these services.
    3. Be innovative and outcomes focussed, seeking to engage with and access the public directly as possible in venues that may not usually be associated with health services.
    4. Respond, under direction, to the identified priorities for the Health Checks Team.
    5. Be informed by, not bound by, the different theoretical models which inform health behaviours, behaviour change and health service access in individuals and population groups.
    6. Participate in and support health information or other campaigns as directed.
    7. Any other delegated duties considered appropriate to the post

You must have an NVQ L3 or equivalent, IT skills and clinical experience working in a similar environment. You must also be willing to undergo further training if necessary.
