Job Description
Temporary hospital discharge facilitator
Social Worker or Community Care Worker
The post is part of the hospital discharge strategy and will work alongside the nurse discharge facilitator to support people to leave hospital in a timely fashion.
Experienced Social Worker or Community Care Worker who has current working knowledge of Younger People with physical disabilities and Older People.
Current working knowledge of Adult Care First 6 systems is essential.
Ideally a full time worker would be preferred, however they will consider part time workers.
The post will be worked across both Honiton and Ottery St Mary hospitals.
