Case Worker - Complex and Critical Care

We currently provide Care Packaging within the fields of Spinal Injuries, ABI, Mental Health, Learning Disabilities, MND, End of Life and Physical Disabilities, and are keen to expand their areas of expertise in an ever changing environment. As a result we are looking to recruit a Case Worker to develop the business in the Belfast area.

At a glance:

Reporting to the Registered Manager, the Case Worker provides a service to clients which includes recruiting support workers and enablement workers to assist individuals to live within their own homes and communities whatever their abilities.

 What you’ll be doing:

• Liaising with Commissioning Managers, Operational Managers and Regional Primary Care Trusts.
• Keeping abreast of current commissioning developments regarding the community healthcare sector.
• Taking active role in recruitment and retention of support workers
• Ensuring high standards of care are achieved in service delivery
• Ensuring all staff have knowledge of current policies, procedures and guidelines in use within the Company
• Risk Assessing & implementing new packages  
• Compiling detailed care plans
• Managing a caseload of Packages of Care from the initial assessment
• Recruiting the staff team through to the day to day running of the package
• Liaising with commissioners, family + friends and all professionals involved in the success of the care.
• Interviewing, supervising, appraising and coordinating support workers
• Encouraging referrals from Associates

 About you:

You will be confident and articulate and able to connect with people at all levels. Ideally you will have experience of working with clients with complex needs. A nursing or social work background or experience of working in the Care sector at Deputy/Team Leader level and case management experience is essential. You will be determined and self motivated, possessing a positive attitude and the drive to succeed.
