A fantastic opportunity for a Case Manager has risen to join our New team in North Yorkshire. The Case Manager will be responsible for recruiting and managing teams of workers in order to deliver a professional and safe service.

Responsibilities will include:

Liaising with Commissioning Managers, Operational Managers and Regional Primary Care Trusts

Keeping abreast of current commissioning developments regarding the continuing healthcare sector.

Taking active role in recruitment and retention of support workers

Ensuring high standards of care are achieved in service delivery

Ensuring all staff have knowledge of current policies, procedures and guidelines in use within the Company

Risk Assessing & implementing new packages

Compiling detailed care plans

You would be expected to manage a caseload of 10 - 15 Packages of Care from the initial assessment, recruiting the staff team through to the day to day running of the package, liaising with commissioners, family + friends and all professionals involved in the success of the care. This is a busy and demanding role working with support workers, clients and colleagues in a lively environment.

Hands on care experience is essential and or previous Case Management, QSW or RN.

Ideally you will have experience of working with clients with complex needs. Anyone with a nursing or social work background or whom has worked in the Care sector at Deputy/Team Leader level and looking for a new challenge would also be welcome to apply.
