Care Manager (Older People)
4Social Work has an exciting Care Manager position that has become available within Older People Services we are seeking a skilled and motivated individual to work within a challenging yet rewarding role for our client based in West London
The position will be initially for 1 month, but is likely to be extended due to the nature of the work.
Duties May Include:
We require a care manager in a Older Peoples care management and review team, you must have good IT skill and extensive experience of working with older people, your main duties will include
• To use social work knowledge and skills to enable clients to live as independently as possible within the limitations of their circumstances and disabilities
• To provide a professional assessment, care management and review service to people with physical and sensory disabilities across the borough
• To safeguard vulnerable adults from abuse
• To work individually, and with others, to achieve key service performance targets
• To use the principles of assessment, care management and review to provide a person centred approach to assessment and care planning, incorporating the needs of carers and working with other disciplines to achieve effective outcomes for service users
• To be accountable for individual performance and to be compliant with data recording requirements
• To work within key legislative requirements and guidance, responding to changing national and local imperatives
• To participate in supervision and in individual appraisal and performance development
Please note that this is a qualified position so you will need a social work degree/masters or equivalent, be GSCC registered and have relevant experience for this role. Any applications received without meeting these minimum criteria will not be considered.
